Wheelchair Seating & Mobility Clinic

 At Vital Kinesis, we provide comprehensive wheelchair and seating evaluations to help individuals improve mobility, comfort, independence, and safety. Our clinic collaborates with experienced seating specialists to ensure each person receives the most appropriate wheelchair and positioning system based on their needs. 


During this evaluation, you will meet with:

A Physical Therapist (PT)

An Assistive Technology Professional (ATP) 


Together, we assess posture, strength, mobility, skin protection needs, and functional goals to determine the best wheelchair and seating options for you.


This clinic is ideal for individuals who:

Currently use a wheelchair 

Have been recommended to obtain a wheelchair 

Need adjustments or replacement equipment 

Are experiencing discomfort, positioning issues, or mobility challenges 


Appointments are held once per month and must be scheduled in advance.

How to get started

Step 1 - Obtain a Referral From Your Doctor

  In most cases, insurance requires a physician referral/prescription for a wheelchair and seating evaluation. This typically must be completed during an in-person visit with your doctor and may not be done via telehealth.

The referral from your physician should include:

  • Your name
  • Date of birth
  • Phone number
  • Address
  • Medical diagnosis
  • Mobility limitations
  • Referral to Physical Therapy for mobility evaluation

Step 2 - Request an Appointment at Vital Kinesis

Our wheelchair seating clinics are held once per month, on the last Friday of each month from 9:00 AM–12:00 PM. Advanced scheduling is required; walk-ins are not accepted.

We will contact you within 24–48 hours to confirm an appointment time. If you are unable to attend during the designated clinic hours, we will do our best to explore alternative options.

Before scheduling, please ensure:

  • You currently use a wheelchair or have been recommended for one
  • An in-person evaluation is preferred
  • You are able to bring your current wheelchair (if applicable)

However, if you unable to come to clinic for an in-person evaluation, we are able to work with you via telehealth. If you are unsure whether this clinic is right for you, please contact us prior to scheduling.

If you are already working with a wheelchair vendor, we would love to know! Our clinic collaborates with National Seating & Mobility.

Step 3 - Complete Intake Forms

  Once your appointment is confirmed, you will receive a patient portal invitation to complete intake and consent forms electronically. All paperwork must be completed prior to your appointment to avoid delays.

Step 4 - Prepare for Your Appointment

  Please bring:

  • Your current wheelchair and seating system (if applicable)
  • Any cushions, backs, or supports you use
  • Relevant medical records or prior wheelchair evaluations (if available)
  • Comfortable clothing that allows posture and movement assessment

Appointments typically last 45–60 minutes, depending on your needs.

If you are participating in the evaluation via telehealth, please make sure you have stable internet connection and access the appointment via the virtual link that was sent to you. 

Step 5 - During the Evaluation

  Your evaluation will include both:

  • A Physical Therapist, and
  • An Assistive Technology Professional (ATP)

The physical therapist will assess posture, mobility, strength, range of motion, balance, coordination, cognition, and skin integrity to determine the most appropriate wheelchair and seating recommendations.


The ATP represents the seating company and has specialized expertise in wheelchair options and configurations. The ATP may also have trial equipment available and may bring to the clinic to help determine the best fit.


This appointment is also an opportunity for you to ask questions and seek clarification. We want you to feel informed and confident in your decisions.

Vital Kinesis collaborates with National Seating & Mobility to provide a comprehensive evaluation experience.

Step 6 - After the Evaluation

 A detailed wheelchair and seating recommendation will be completed. Your physical therapist and      ATP will prepare required documentation and justification. National Seating & Mobility will submit materials to your physician and insurance provider. National Seating & Mobility will coordinate equipment ordering, updates, delivery, and any follow-up services.


National Seating & Mobility will be in touch with you and be your main point of contact for any questions and/or follow-up services.
